Subjective: 74F admitted with pyelonephritis, now afebrile for 48 hours with resolved flank pain. Objective: Hemodynamically stable, tolerating a regular diet, ambulating independently. Assessment/Plan: Pyelonephritis improved; transitioned from IV ceftriaxone to oral cefpodoxime with a clear plan to complete the course as an outpatient. Ready for discharge home.
