Subjective: 81F post-op day 3 from ORIF of a right hip fracture, pain well controlled on oral analgesia. Objective: Incision clean and dry, afebrile. Assessment/Plan: Cleared by PT for transfer with assist; VTE prophylaxis in place. Appropriate for acute rehab.
